Shadow Visit Request

The Shadow Visit allows you the opportunity to observe animal-assisted therapy at Lend-A-Heart
facilities and participate with the teams and clients. Do not bring your pet to the visit.

A prospective team must attend one shadow visit prior to testing. If you have any questions or
comments send an e-mail to info@google.com

Please complete the below form with your information, choose a facility and send. The LAH team
leader for the facility will contact you.
